Be prepared for...

Your Appointment with the Attorney


Your Estate:

Make a list of your assets with full legal descriptions for each parcel of real estate (e.g., copy of deed or mortgage). If you have a business (corporation, LLC, association) bring the paperwork and be able to discuss the way it is set up.


Distribution of Your Assets:

How do you want your estate distributed? Be specific - your child could die before you with no will. If a charity is to get anything, bring its full legal name.


Your Will:

Bring a copy of your most recent will. Who would you designate as guardian of minor children if both parents die? Who should "administer" your estate in Probate?


Durable Power of Attorney:

Who would you appoint to handle your affairs if you are absent or incapacitated?


Living Will Declaration:

If your medical condition is terminal and you are being sustained artificially, would you want life support withdrawn? Who would authorize that? Who would you respectfully decline to authorize? Discuss your wishes with the persons you select.


Health Care Power of Attorney:

Who will you authorize to make routine medical decisions? Are there any specific instructions (e.g., allergies, no blood transfusions)?
